 Post subject: PHP email form and mysql entry

hello

I need some guidance. Basically I want to create a php form that will
accept user input for orders.

Once they click submit, this information will inserted into a MySQL database and the same information emailed to me.

Ideally I would like to tie this into a login management system for my users, so that users will have to login to access the order form, and some field will be automatically populated based on the logged in user.

I am on a freehostia paid account.

have you tried oscommerce?

it's in the Elefante Installer under E-COMMERCE
does everything you've asked plus more.
